Employee and Personnel Forms for your business

About Personnel Files - While no law specifically requires you to maintain personnel files for your employees, they're a good way of keeping track of the employee information you are required to retain (except for confidential information, such as medical records, of course).

This way, you can be sure that important documents are kept for the proper period of time, and you'll be able to find them should they ever be requested by a federal agency - or you need them to defend against an employee lawsuit.

Use these forms to chronicle why an employee was absent, how long he/she was absent, and how this leave will be accounted for (i.e., deducted from accrued sick leave, etc.).


Use to tally an employee's year-end absence total. Be sure to note any and all periods of leave, even if it was excused or not counted against an employee's accrued time off.


Keep track of company property by having employees who borrow company items initial this checklist.


After disciplining an employee for misconduct, complete this form and be sure to obtain his/her signature. And remember, having the employee sign this form is not a substitution for fully explaining the reasoning behind your discipline decision.

Each time an employee requests to be transferred, fill out this form, paying special attention to the employee qualifications and the reasons for the request.


Soon after an employee is hired, have him/her complete one of these forms. They provide basic information, like who to contact in case of an emergency, spouse's name, etc.


Use this to keep track of an employee's job status, and whether or not that status changes.


Getting an employee's take on his/her job responsibilities will help you to create job descriptions and identify an employee's essential duties.


Have all employees who request a leave of absence fill out this form, which documents the type of leave requested, the period of leave requested, and the reason for the request.
